As a child, Sasha grew up in the sort of environment in which it was more beneficial to him to lie constantly than it was to either tell the truth or to just not do things he would have to lie about in the first place. He was abandoned by his parents around age twelve, most likely because of the medical care he required, and became a ward of the state. As far as he is concerned, this is one of the best things that happened to him because he was able to finish school and because it meant his health was monitored faithfully, so he didn't spend most of this time feeling unwell.
Sasha's health issues are a genetic part of his albinism. He has a condition that is contermporarily called Hermansky-Pudlak Syndrome (HPS), which can involve bleeding problems as in excessive bleeding with trouble clotting. Though his HPS is generally mild, Sasha often develops spontaneous nosebleeds, and he bruises easily and badly. Another aspect of HPS that affects him is pulmonary fibrosis, which means that the tissue of the lung scars, and restricts the inflation of the lungs, worsening over time. This has led to general upper-respiratory problems that recur chronically if he doesn't take care of himself. In addition, though less medically problematic, he has a condition called nystygmus, or involuntary rapid eye movements; he is unaware of the movements himself and they are so slight and fast that most people don't notice them unless they are paying close attention.
Fortunately, the city foots the bill for any medical treatments he requires, as well as for the pills he takes to combat the side effects of lacking skin pigmentation. None of this, however, has done anything to combat the fact that Sasha is a compulsive liar and a co-dependent person. By befriending and becoming involved in a relationship with Sasha, Julian has received the brunt of the effects of these two aspects of Sasha's personality. Sasha's neediness has only been exacerbated by his inheritance of the clairvoyant powers of the Witness, which for him manifest mainly as visual, somewhat hallucinatory, specters. Intensely jealous of Surge and, to a lesser degree, Rabbit, Sasha clings to Julian and demands his full attention when he feels his claim is being threatened.
